About this policy
This policy is published by Westhaven Venture LLC, operating Trusted Diabetic Help, for the Washington My Health My Data Act (RCW 19.373) and similar state consumer health data laws. It covers consumer health data we collect through trustediabetichelp.com and our program. Our Privacy Policy covers everything else.
1. The consumer health data we collect
When you ask for an offer, the form collects the items you list. Because those items are diabetes supplies, they can be consumer health data. They include:
- The supplies you list on the form: the type of supply, such as blood glucose test strips or CGM sensors, the brand, and the box count
- Details about those supplies that you send us, such as expiration dates, condition, and photos
- What our team records when your shipment is inspected
- Any inference that could be drawn from these items, such as that someone in your household has diabetes
We do not ask for a diagnosis, a prescription, insurance details, or medical records, and we do not want them.
2. Why we collect it
We collect consumer health data only to make you an offer, issue your prepaid shipping label, inspect your shipment, pay you, keep the records the law requires, and answer your questions. We collect it with your consent, which you give when you submit the form, and we collect no more than the program needs.
3. Where it comes from
Directly from you: what you type into our form, what you send our team by text, email, or phone, and the shipment you send us.
4. Who receives it
Consumer health data stays with Westhaven Venture LLC and its service providers only. Our service providers process it on our behalf, under contract, to run the program: our website host and its form service, our customer messaging and records systems, and Shippo, our shipping platform. We do not share consumer health data with any other third party or with any affiliate.
5. Never sold
We never sell consumer health data, and we never share it for advertising. This site runs no advertising code, no conversion tracking, and no analytics of any kind.
6. Your rights
You may ask us to confirm whether we hold your consumer health data, to give you a copy, to tell you every third party and affiliate it went to, to withdraw your consent, and to delete it. We will not treat you differently for asking.
7. How to ask for deletion
Email support@trustediabetichelp.com with the subject line Consumer health data request, call (973) 306-9933, or write to the address below. Tell us the name, email, or phone number you gave us and say that you want your consumer health data deleted. We will confirm the request is yours, delete the data from our records and tell our service providers to delete it from theirs, and answer within 45 days. If we decline any part of a request, we will tell you why and how to appeal, and you may contact the Washington State Attorney General if you are not satisfied with the appeal.
